Japanese Persimmon and Cilazapril

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIHYPERTENSIVE DRUGS

Why this applies to Cilazapril

Cilazapril falls under ANTIHYPERTENSIVE DRUGS — the group Japanese Persimmon interacts with.

Japanese persimmon is thought to have hypotensive effects. Theoretically, concurrent use might increase risk of hypotension with drugs that lower blood pressure. These include captopril (Capoten), enalapril (Vasotec), losartan (Cozaar), valsartan (Diovan), diltiazem (Cardizem), amlodipine (Norvasc), hydrochlorothiazide (HydroDiuril), furosemide (Lasix), and many others.
Severity Moderate Likelihood Probable Evidence B

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Cilazapril is an ACE inhibitor that relaxes your blood vessels to bring blood pressure down. Japanese persimmon, especially in concentrated supplement or leaf-extract form, may have its own mild blood-pressure-lowering action. So when you pair the two, the worry is simple addition: your pressure could drop a bit more than expected.

If that happens, you might feel dizzy or lightheaded, especially when you stand up quickly, and some people feel tired or wobbly. Keep in mind this is mostly theoretical, based on limited research, and eating persimmon as a regular fruit is generally fine. It's the concentrated products that raise the most concern. Don't change anything on your own, but do check with your pharmacist or doctor first.

From our pharmacist’s overview on the Cilazapril page

“Cilazapril (Inhibace) is an ACE inhibitor for high blood pressure, and its supplement list warrants genuine care rather than a quick skim. Most entries either add to its blood-pressure-lowering effect (fish oils, celery seed extract, many others) or pull against it, the way tyramine can drive pressure up and blunt the drug.”

Japanese persimmon is a nutritious fruit rich in fiber, vitamins, and antioxidant plant compounds, and its leaves and calyx are used in traditional medicine.
