Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group Japanese Persimmon interacts with.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing with Iloprost (Ventavis): it's already a vasodilator, meaning its whole job is to relax and widen your blood vessels to ease the pressure in your lungs. That can lower your overall blood pressure as a side effect. Japanese persimmon is thought to gently lower blood pressure too, so stacking the two together could add up and push your pressure down a little more than expected.
In real life that might show up as feeling dizzy, lightheaded, faint, or unusually tired, especially when you stand up quickly. To be clear, this is a theoretical, modest concern, and eating persimmon as a normal food is far less likely to matter than concentrated leaf extracts or supplement capsules. Still, since PAH is serious, please run any persimmon supplement past your pharmacist or doctor first.

Some anticoagulant and antiplatelet drugs include aspirin, clopidogrel (Plavix), dalteparin (Fragmin), enoxaparin (Lovenox), heparin, indomethacin (Indocin), ticlopidine (Ticlid), warfarin (Coumadin), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the connection worth knowing. Iloprost (Ventavis) relaxes your blood vessels to treat pulmonary hypertension, but it also has a mild blood-thinning, anti-platelet effect, which is why it gets grouped with drugs like aspirin and warfarin. Lab studies suggest certain proteins in persimmon leaf extracts may slow clotting too. Put the two together and the theory is that bleeding could become a little easier, meaning more bruising, nosebleeds, or bleeding gums.
I want to be honest about the strength here: this is theoretical only, seen in test tubes and never proven in people. Eating the fruit as a food is very unlikely to matter. Concentrated leaf extracts or supplements are the bigger question mark, so check with your pharmacist or doctor before combining them.
