Theoretically, khella might have additive adverse effects on the liver when used with hepatotoxic drugs. Khella can increase liver transaminases. Some drugs that can adversely affect the liver include acetaminophen (Tylenol), amiodarone (Cordarone), carbamazepine (Tegretol), isoniazid (INH), methotrexate (Rheumatrex), methyldopa (Aldomet), and many others.

Here's the concern with pairing khella and Vadadustat (Vafseo). Vadadustat, which is used to treat anemia in chronic kidney disease, can already put some stress on the liver and raise liver enzymes for certain people. Khella has also been linked to bumps in liver enzymes (transaminases). Put the two together and the worry is that those effects could add up, making liver irritation a bit more likely.

I want to be honest that this is mostly theoretical and based on limited evidence, so it isn't a reason to panic. Still, since both can nudge liver numbers in the same direction, please loop in your pharmacist or doctor before combining them. They can keep an eye on your liver bloodwork and watch for signs like yellowing skin, dark urine, belly pain, or unusual tiredness.
