L-arginine + Cilazapril
Cilazapril brand name: Inhibace.
Cilazapril falls into 2 drug categories that L-arginine can interact with; the most serious is rated moderate. Theoretically, concomitant use of L-arginine and antihypertensive drugs may increase the risk of hypotension.

These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
L-arginine interacts with whole categories of medication. Cilazapril falls into 2 of those categories, so it can interact with L-arginine in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
ANTIHYPERTENSIVE DRUGS
1 of 2Why this applies to Cilazapril
Cilazapril falls under ANTIHYPERTENSIVE DRUGS — the group L-arginine interacts with.
L-arginine increases nitric oxide, which causes vasodilation. Clinical evidence shows that L-arginine can reduce blood pressure in some individuals with hypertension. Furthermore, combining L-arginine with some antihypertensive drugs seems to have additive vasodilating and blood pressure-lowering effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to know if you pair these two. Cilazapril is an ACE inhibitor that lowers your blood pressure, partly by relaxing your blood vessels. L-arginine works on a similar pathway, since your body turns it into nitric oxide, which also widens vessels. Stack them together and the blood-pressure-lowering effect can add up.
This is mostly a theoretical concern based on how the two work, so for many people it won't cause trouble. But if you notice dizziness, lightheadedness when standing, headache, or feeling faint, that can be a sign your pressure dropped too low. Don't change anything on your own. Just check with your pharmacist or doctor first, especially if you have heart issues.

ACE INHIBITORS (ACEIs)
2 of 2Cilazapril also falls under ACE INHIBITORS (ACEIs).
Combining L-arginine with some antihypertensive drugs, especially ACE inhibitors, seems to have additive vasodilating and blood pressure-lowering effects. Furthermore, ACE inhibitors can increase potassium levels. Use of L-arginine has been associated with hyperkalemia in some patients. Theoretically, concomitant use of ACE inhibitors with L-arginine may increases the risk of hyperkalemia.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Cilazapril (Inhibace) is a blood pressure medicine that relaxes your blood vessels and can nudge your potassium levels up a bit. L-arginine works in a similar direction by helping make nitric oxide, which also widens blood vessels. So if you take them together, the two can stack on top of each other.
The realistic concern here is that your blood pressure could dip a little lower than expected, leaving you feeling dizzy, lightheaded, or tired, especially when standing up. There's also a theoretical chance your potassium could climb higher. I'll be honest, this interaction is based on how these work in the body rather than strong studies. Still, it's worth a quick chat with your pharmacist or doctor before adding L-arginine.

From our pharmacist’s overview on the Cilazapril page
“Cilazapril (Inhibace) is an ACE inhibitor for high blood pressure, and its supplement list warrants genuine care rather than a quick skim. Most entries either add to its blood-pressure-lowering effect (fish oils, celery seed extract, many others) or pull against it, the way tyramine can drive pressure up and blunt the drug.”

About L-arginine
What is L-arginine?
- High blood pressure
- Erectile dysfunction
- Athletic performance
- Chest pain (angina)
- Improving blood flow
L-arginine is an amino acid that the body uses to make nitric oxide, a substance that helps blood vessels relax and widen. It is popularly used for blood pressure, erectile dysfunction, and exercise, but the evidence is mixed and stronger for some uses than others. It is generally well tolerated in healthy adults, but talk with a pharmacist or doctor before using it, especially if you have heart problems or take other medicines.
