Why this applies to Methenamine Hippurate
Methenamine Hippurate falls under ANTIBIOTIC DRUGS — the group Lacticaseibacillus Rhamnosus interacts with.
L. rhamnosus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. rhamnosus preparations by at least two hours.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind: this interaction goes in one direction, and it's about the probiotic, not your antibiotic. Methenamine hippurate (Hiprex, Urex) won't become weaker or stronger because of L. rhamnosus. It works by releasing formaldehyde in acidic urine to keep urinary tract infections at bay, and the probiotic doesn't change that.
What can happen is the reverse. Since methenamine is an antibacterial, taking it at the same time as a live probiotic may kill off some of those beneficial bacteria, so you get less benefit from the L. rhamnosus. This is theoretical, based on how the two work, not proven harm. A simple fix is to space them out by at least two hours. If you're unsure, your pharmacist can help you set up a schedule.

What Is Methenamine Used For?
Brand names: Urex, Hiprex
Methenamine is used to suppress or prevent frequently recurring urinary tract infections (UTIs), including pyelonephritis (kidney infection) and cystitis (bladder infection). It is intended for long-term or ongoing use after another antibiotic has already cleared the active infection — not as a first treatment for a new, acute infection.
Methenamine Mandelate (available as an oral solution and tablet) is also indicated for infected residual urine that can occur with certain neurologic diseases. It is not appropriate as the sole treatment when an infection is causing systemic symptoms like chills and fever.
