Methenamine Hippurate falls under ANTIBIOTIC DRUGS — the group Bifidobacterium Bifidum interacts with.

Theoretically, taking Bifidobacterium. bifidum with antibiotic drugs might decrease the effectiveness of B. bifidum.
Since B. bifidum preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. bifidum preparations by at least 2 hours.

Here's the simple version: this interaction isn't about the medicine being harmed. Methenamine hippurate works just fine. Instead, the concern runs the other way. Methenamine is an antibiotic that releases formaldehyde in your urinary tract to clear out infection-causing bacteria, and it can also kill some of the friendly bacteria in your probiotic. So your Bifidobacterium bifidum supplement may simply do less for you.

The good news is this is a theoretical worry based on how the two work, not strong real-world data. An easy fix is to space them out, taking your probiotic about 2 or more hours apart from your methenamine dose. Your UTI treatment stays effective, and your probiotic gets a better chance to survive. When in doubt, run the timing by your pharmacist.

About Methenamine

What Is Methenamine Used For?

Brand names: Urex, Hiprex

Methenamine is used to suppress or prevent frequently recurring urinary tract infections (UTIs), including pyelonephritis (kidney infection) and cystitis (bladder infection). It is intended for long-term or ongoing use after another antibiotic has already cleared the active infection — not as a first treatment for a new, acute infection.

Methenamine Mandelate (available as an oral solution and tablet) is also indicated for infected residual urine that can occur with certain neurologic diseases. It is not appropriate as the sole treatment when an infection is causing systemic symptoms like chills and fever.
