Methenamine Hippurate falls under ANTIBIOTIC DRUGS — the group Bacillus Subtilis interacts with.

Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.

Here's the thing to know about pairing Bacillus subtilis with methenamine hippurate (Hiprex, Urex): the worry runs in one direction only. Because methenamine works by releasing formaldehyde in the urinary tract to kill off bacteria, it can also kill the live probiotic organisms in your Bacillus subtilis supplement if they're in your system at the same time. So the supplement may simply do less for your gut, not the other way around.

Your UTI prevention from methenamine should still work fine. This is a theoretical concern based on how the two work, not strong proof. If you take both, a simple fix is to space them out by at least 2 hours. Check with your pharmacist if you're unsure about timing.

About Methenamine

What Is Methenamine Used For?

Brand names: Urex, Hiprex

Methenamine is used to suppress or prevent frequently recurring urinary tract infections (UTIs), including pyelonephritis (kidney infection) and cystitis (bladder infection). It is intended for long-term or ongoing use after another antibiotic has already cleared the active infection — not as a first treatment for a new, acute infection.

Methenamine Mandelate (available as an oral solution and tablet) is also indicated for infected residual urine that can occur with certain neurologic diseases. It is not appropriate as the sole treatment when an infection is causing systemic symptoms like chills and fever.
