Bacillus Subtilis and Rifampin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Rifampin

Rifampin falls under ANTIBIOTIC DRUGS — the group Bacillus Subtilis interacts with.

Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing to understand about this pairing: it works the other way around from what most people worry about. Rifampin is a powerful antibiotic, and Bacillus subtilis is a live probiotic. Rifampin doesn't get weaker or stronger from the probiotic. Instead, the antibiotic can kill off a good chunk of those live, beneficial organisms before they get a chance to do their job in your gut. So the supplement is the one that loses out here, not your TB or meningitis treatment.

The good news is this is easy to manage and is based on how antibiotics work, not on hard studies. A simple fix is to space them out by at least 2 hours. Keep taking your rifampin exactly as prescribed, and check with your pharmacist about timing.

About Bacillus Subtilis

What is Bacillus Subtilis?

  • Digestive health and bloating
  • Diarrhea support
  • Immune support
  • Gut microbiome balance

Bacillus subtilis is a spore-forming probiotic bacterium found in soil and some fermented foods, and it is used mainly to support gut health and digestion. Early research is promising for certain digestive and immune uses, but evidence is still limited and not strong enough to make firm claims. Talk to a pharmacist or doctor before using it, especially if you have a weak immune system.

About Rifampin

What Is Rifampin Used For?

Brand names: Rifadin, Rimactane, Rifadin Injection, Rofactor

Rifampin treats all forms of tuberculosis. It is always used as part of a combination of TB medicines, not on its own.

It also treats asymptomatic meningococcal carriers. These are people who carry the bacteria Neisseria meningitidis in the nose and throat without being sick. Rifampin is not used to treat an actual meningococcal infection.

Rifampin capsules are taken by mouth. Rifadin IV and Rifampin for Injection are given through a vein for the initial treatment and retreatment of tuberculosis when the medicine cannot be taken by mouth.
