Bacillus Subtilis and Oxacillin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Oxacillin

Oxacillin falls under ANTIBIOTIC DRUGS — the group Bacillus Subtilis interacts with.

Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: this one runs in the opposite direction from most drug interactions. Oxacillin won't be harmed by Bacillus subtilis. Instead, the antibiotic may weaken the probiotic. Oxacillin is a penicillin, meaning its whole job is to kill bacteria, and it can't always tell the difference between the bug causing your infection and the friendly spores in your supplement.

So if you take both at the same time, you may simply be wasting the probiotic. This is a theoretical concern based on how each one works, not something proven to cause harm. A common, easy fix is to space them out by at least 2 hours. Your oxacillin still works as expected either way. If you're unsure how to time them, your pharmacist can map it out for you.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Oxacillin page

“Kidney harm is the first risk to understand with oxacillin (Bactocill, Prostaphlin), a penicillin antibiotic given for serious staph infections. Siberian cocklebur and asarabacca can each damage the kidneys on their own, and combining them with oxacillin risks compounding that harm; both carry major ratings, as does activated charcoal,…”

About Bacillus Subtilis

What is Bacillus Subtilis?

  • Digestive health and bloating
  • Diarrhea support
  • Immune support
  • Gut microbiome balance

Bacillus subtilis is a spore-forming probiotic bacterium found in soil and some fermented foods, and it is used mainly to support gut health and digestion. Early research is promising for certain digestive and immune uses, but evidence is still limited and not strong enough to make firm claims. Talk to a pharmacist or doctor before using it, especially if you have a weak immune system.

About Oxacillin Injection

What Is Oxacillin Injection Used For?

Brand names: Prostaphlin, Bactocill

Oxacillin Injection treats infections caused by penicillinase-producing staphylococci. These are staph bacteria that make an enzyme that destroys regular penicillin. Lab tests should show the bacteria are susceptible to oxacillin.

Doctors may start it before test results are back if a resistant staph infection is suspected. It should not be used for infections caused by bacteria that penicillin G can treat. It should be stopped if tests show the infection is not caused by a resistant staph.
