This product contains 4 active ingredients—each a live, spore-forming probiotic microorganism. Saccharomyces boulardii is a beneficial yeast; Bacillus coagulans (branded as LactoSpore) and Bacillus subtilis (DE111) are beneficial bacteria; and Bacillus clausii (SC-109) is another bacterial strain.
These organisms are intended to support digestive health by colonizing your gut. The capsules also contain two inactive ingredients—guar gum and hydroxypropyl methylcellulose—as binders and stabilizers.
Does it work?
Moderate evidence
Saccharomyces boulardii is possibly effective for rotaviral diarrhea, travelers' diarrhea, antibiotic-associated diarrhea, necrotizing enterocolitis, Clostridioides difficile infection, and Helicobacter pylori infection. Bacillus coagulans is possibly effective for constipation and irritable bowel syndrome, though the evidence for antibiotic-associated diarrhea is insufficient to rate.
Bacillus subtilis is possibly effective for antibiotic-associated diarrhea; evidence for athletic performance, cirrhosis, constipation, atopic dermatitis, and diabetes is insufficient. We hold no effectiveness data for Bacillus clausii.
How safe is it?
Well-documented data
All three organisms we could check are generally well tolerated in healthy adults, though safety data varies by product and strain, and you should talk with your doctor before using. The key safety concern is that these are live organisms: in critically ill or immunocompromised patients—especially those with central venous catheters, on broad-spectrum antibiotics, or receiving tube feeding—Saccharomyces boulardii has rarely caused bloodstream infection (fungemia).
Similarly, bacteremia and sepsis have been reported with other live probiotics in severely ill or immunocompromised patients. Gastrointestinal side effects from S. boulardii are rare but include cramps, gas, nausea, vomiting, and loss of appetite; one headache was reported with Bacillus subtilis, and diarrhea has been reported rarely.
Pregnancy and lactation safety data are not on file for these ingredients.

With antifungals, the concern is direct: since S. boulardii is a live yeast, antifungal medications may eliminate a significant portion of it. With antibiotics—which affect both Bacillus coagulans and Bacillus subtilis—the issue is similar: the drugs kill bacteria indiscriminately, including the beneficial strains in this product.

LactoSpore
Antibiotic Drugs
Theoretically, taking antibiotics with Bacillus coagulans might decrease the effectiveness of B. coagulans.
B. coagulans preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. coagulans preparations by at least two hours.
Bacillus subtilis (DE111)
Antibiotic Drugs
Theoretically, taking Bacillus subtilis with antibiotic drugs might decrease the effectiveness of B. subtilis.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
Saccharomyces boulardii
Antifungals
Theoretically, taking antifungals with Saccharomyces boulardii might decrease the effectiveness of Saccharomyces boulardii.
S. boulardii is a live yeast. Therefore, simultaneously taking antifungals might kill a significant number of the organisms.
