ANTIBIOTIC DRUGS
Why this applies to Nitrofurantoin
Nitrofurantoin falls under ANTIBIOTIC DRUGS — the group Bacillus Subtilis interacts with.
Since B. subtilis preparations usually contain live and active organisms, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and B. subtilis preparations by at least 2 hours.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind with this pair: it's the probiotic that takes the hit, not your antibiotic. Nitrofurantoin works by killing bacteria in your urinary tract, and it can't tell the difference between the bugs causing your infection and the friendly Bacillus subtilis spores you swallowed. So if you take them at the same time, the antibiotic may wipe out a good chunk of the probiotic before it can do its job.
Your nitrofurantoin will still treat your UTI just fine, so don't change how you take it. If you want the probiotic to have a fair shot, simply space the two at least 2 hours apart. This is based on how the drugs work rather than hard studies, so it's a sensible precaution, not a danger. Ask your pharmacist if you're unsure about timing.

From our pharmacist’s overview on the Nitrofurantoin page
“Nitrofurantoin (Macrobid, Macrodantin) treats urinary tract infections and can strain the liver on its own, which is why this interaction list deserves genuine care rather than a glance. Of the 82 ingredients on file, 38 can add their own liver stress on top of the drug's, and three of the four major-rated entries fall in that group, i…”

About Bacillus Subtilis
What is Bacillus Subtilis?
- Digestive health and bloating
- Diarrhea support
- Immune support
- Gut microbiome balance
Bacillus subtilis is a spore-forming probiotic bacterium found in soil and some fermented foods, and it is used mainly to support gut health and digestion. Early research is promising for certain digestive and immune uses, but evidence is still limited and not strong enough to make firm claims. Talk to a pharmacist or doctor before using it, especially if you have a weak immune system.
About Nitrofurantoin
What Is Nitrofurantoin Used For?
Brand names: Macrodantin, Furadantin, Furatoin, Macrobid
Nitrofurantoin treats urinary tract infections (UTIs) caused by specific susceptible bacteria. The capsule forms (including Macrodantin and nitrofurantoin macrocrystals capsules) are indicated for UTIs due to Escherichia coli, enterococci, Staphylococcus aureus, and certain strains of Klebsiella and Enterobacter species. Nitrofurantoin capsules (monohydrate/macrocrystals) are indicated only for acute uncomplicated UTIs (acute cystitis) caused by E. coli or Staphylococcus saprophyticus.
Nitrofurantoin oral suspension (Furadantin) is indicated for adults and children 1 month of age and older for UTIs caused by susceptible strains of E. coli, Enterococcus species, S. aureus, Klebsiella species, and Enterobacter species. No form of nitrofurantoin is approved for kidney infections (pyelonephritis) or infections in the tissue surrounding the kidneys (perinephric abscesses).
