Lactobacillus acidophilus and Lactobacillus helveticus are generally well tolerated in healthy people and children, though caution is advised — especially for anyone seriously ill or immunocompromised. Bacillus subtilis is also generally well tolerated in healthy adults, but quality and safety vary by product and strain.

All three live-organism probiotics carry a theoretical risk of infection in vulnerable patients; most documented cases happened in people with serious underlying conditions. Common side effects from probiotics are mild and uncommon: epigastric discomfort, abdominal pain, indigestion, gas, bloating, diarrhea, vomiting, and burping.

One trial of a probiotic blend reported rash and itching in a small number of participants, though it's unclear whether L. acidophilus caused it. Rare cases of headache and diarrhea have been reported with Bacillus subtilis.

Antibiotic Drugs

Theoretically, taking Lactobacillus acidophilus with antibiotic drugs might decrease the effectiveness of L. acidophilus.
L. acidophilus preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. acidophilus preparations by at least two hours.
