Lactiplantibacillus Plantarum and Pentamidine

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTIBIOTIC DRUGS

Why this applies to Pentamidine

Pentamidine falls under ANTIBIOTIC DRUGS — the group Lactiplantibacillus Plantarum interacts with.

Theoretically, taking Lactiplantibacillus plantarum with antibiotic drugs might decrease the effectiveness of L. plantarum.
L. plantarum preparations usually contain live and active organisms. Therefore, simultaneously taking antibiotics might kill a significant number of the organisms. Tell patients to separate administration of antibiotics and L. plantarum preparations by at least two hours.
Severity Moderate Likelihood Probable Evidence D

Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the key thing to know: this interaction works in one direction only. Pentamidine is a strong medicine given for a serious lung infection, and your probiotic won't make it work better or worse, and it won't make pentamidine's side effects (like low blood sugar, kidney strain, or blood pressure changes) any stronger. Pentamidine isn't a prodrug, so there's no concern about it building up or weakening.

What can happen is the opposite: pentamidine is given to fight germs, and it can also kill off the live, friendly bacteria in your L. plantarum supplement, so you may get less benefit from the probiotic. This is theoretical, based on how the two work, not strong proof. If you take both, simply space them at least two hours apart and check with your pharmacist.
