Here's the gist with Synalgos DC (aspirin, caffeine, and dihydrocodeine) and lemongrass. The dihydrocodeine part leans on liver enzymes, including CYP3A4, for part of its breakdown. Lemongrass — really its compound citral — has slowed down that same enzyme in lab and animal studies. In theory, that could let a bit more dihydrocodeine linger, which might mean more drowsiness, dizziness, constipation, or slowed breathing. Lemongrass may also nudge up caffeine levels, so you might feel jittery.

I want to keep this in perspective: this is only seen in test-tube and animal research, not in people, and a splash of lemongrass tea or seasoning in food is very unlikely to matter. Concentrated supplements or the essential oil are a different story — run those by your pharmacist or doctor first.
