Bedaquiline falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lemongrass interacts with.

Theoretically, lemongrass might decrease the metabolism of CYP3A4 substrates.
Animal research shows that lemongrass and its constituent citral inhibit CYP3A4.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to break down and clear bedaquiline. Lab and animal studies hint that lemongrass — and its lemony compound citral — may slow that enzyme down. If that happened in people, bedaquiline could clear more slowly and build up a bit higher than expected. With this drug, the main concern would be its effect on heart rhythm (a QT prolongation that shows up on an EKG), plus possible nausea or liver irritation.

That said, this is theoretical — it comes from test-tube and animal work, not human studies, and the lemongrass in a cup of tea or your cooking is unlikely to matter. Concentrated supplements or the essential oil are a different story. Since MDR-TB treatment is serious, please run any lemongrass supplements by your TB team or pharmacist first.

What Is Bedaquiline Used For?

Brand name: Sirturo

Sirturo (bedaquiline) is used as part of a combination treatment for pulmonary TB — TB that affects the lungs — caused by bacteria resistant to at least both rifampin and isoniazid. It is approved for adults and for children 2 years and older who weigh at least 8 kg (about 17.6 lbs).

Sirturo is not used for latent TB (TB infection with no active symptoms), TB outside the lungs, drug-sensitive TB, or infections caused by non-tuberculosis mycobacteria. It must always be used alongside other TB medicines — never alone.
