Here's the gist for irinotecan (Onivyde). Your body relies on a liver enzyme called CYP3A4 to help process this chemo drug. Lab and animal studies suggest that compounds in lemongrass, especially citral, may slow that enzyme down. In theory, that could let more active irinotecan linger in your system, which might mean stronger side effects like severe diarrhea, low white blood cell counts, nausea, and fatigue.

I want to be honest: this is a theoretical concern based only on lab and animal work, not on human studies. The lemongrass in your tea or cooking is unlikely to matter much. Concentrated supplements or the essential oil are a different story. Since you're on chemotherapy, please run any lemongrass supplement past your oncology team first.

Theoretically, lemongrass might increase the clearance and decrease the levels of glucuronidated drugs.
Animal research shows that lemongrass and its constituent citral induce uridine diphosphoglucuronosyl transferase (UGT), the major phase 2 enzyme that is responsible for glucuronidation.

Here's the idea behind this one. Your body partly gets rid of irinotecan (and its active form, SN-38) by attaching a sugar molecule to it — a process called glucuronidation, handled by an enzyme group known as UGT. In lab and animal studies, lemongrass and its main compound citral seem to rev up that enzyme. In theory, a faster cleanup could lower the active drug levels and make your chemotherapy work less effectively against the cancer.

I'll be straight with you: this is theoretical and based only on lab and animal data, not human studies, so we don't really know if it matters in real life. Still, with a serious cancer drug like this, I wouldn't add concentrated lemongrass supplements or essential oil without checking first. An occasional cup of lemongrass tea or its use as a cooking herb is far less concerning. Please run any supplement past your oncology team before starting.
