Why this applies to Thiotepa
Thiotepa falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Lemongrass interacts with.
Animal research shows that lemongrass and its constituent citral inhibit CYP3A4.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the wrinkle with thiotepa: it's actually a prodrug. Your body uses the CYP3A4 enzyme to convert thiotepa into its active form (a compound called TEPA) that does the cancer-fighting work. Lemongrass, and especially its compound citral, has been shown in lab and animal studies to slow down that same CYP3A4 enzyme.
So in theory, concentrated lemongrass could mean less of the drug gets activated, which might make a carefully dosed chemo treatment work less well. The flip side is that more of the original thiotepa could linger, which could shift its side effects too. Honestly, this is based on early lab data, not human studies, so it's far from proven. Cooking with lemongrass or sipping the tea is unlikely to matter, but please run any concentrated supplements or essential oil past your oncology team first.

It is generally considered safe in the small amounts used in food, but most of its claimed health benefits come from laboratory or animal studies rather than strong human research. About Thiotepa Injection
What Is Thiotepa Injection Used For?
Brand names: Thioplex, Tepadina
Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.
Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
