Here's the short version: this isn't about how your body breaks down losartan, and it won't make your blood pressure medicine stronger or weaker. The concern is something different called phototoxicity, which simply means your skin may sunburn more easily. Losartan is grouped with medicines that can make skin a bit more sensitive to sunlight, and concentrated lime oil on the skin can do the same thing.

Now, the honest part: this warning comes only from lab and animal studies, so it's theoretical and quite mild. Eating a lime or squeezing one in your drink is very unlikely to cause any trouble. The bigger caution is putting lime oil or juice directly on your skin and then heading into bright sun. If you spend lots of time outdoors, wear sunscreen and ask your pharmacist if you have questions.

Animal research shows that phenolics in lime juice inhibit cytochrome P450 3A4. This has been demonstrated in rats treated with sildenafil, a CYP3A4 substrate, and high doses of lime juice.

Here's a wrinkle worth knowing about losartan. Unlike most blood pressure pills, losartan is actually a prodrug. Your liver uses an enzyme called CYP3A4 (along with CYP2C9) to convert it into its more powerful active form, which does most of the heavy lifting to lower your blood pressure.

The research on file suggests that compounds in lime juice may slow down that CYP3A4 enzyme. In theory, that could mean less of the active form is made, so losartan might not lower your pressure quite as well. That said, this comes only from animal studies using high doses of lime juice, so it's far from proven, and a squeeze of lime on your food is very unlikely to matter. If you're using concentrated lime products or notice your blood pressure creeping up, just check in with your pharmacist or doctor.

About Losartan

What Is Losartan Used For?

Brand name: Cozaar

Losartan tablets — available as branded Cozaar and several generic products — are used to treat high blood pressure (hypertension) in adults and children 6 years and older. Lowering blood pressure reduces the risk of strokes and heart attacks. Losartan tablets are also used to reduce the risk of stroke in people with high blood pressure who also have left ventricular hypertrophy (an enlarged left side of the heart). Note: evidence suggests this stroke-risk benefit may not apply to Black patients.

Losartan tablets are additionally approved to protect the kidneys in people with type 2 diabetes, high blood pressure, and signs of kidney damage (diabetic nephropathy). Arbli, the oral suspension form, shares these same approved uses.
