This combination pill pairs hydralazine with reserpine, and both ingredients work to lower your blood pressure. Nattokinase isn't broken down or activated by them, so this isn't an enzyme issue. Instead, the concern is simply additive: nattokinase may nudge your blood pressure down a little on its own, and stacking that on top of two blood pressure medicines could push it lower than intended.

If that happens, you might feel dizzy, lightheaded, or faint, especially when standing up quickly. To be clear, this is a theoretical caution based on how these work together, not a strongly proven problem. Still, since your medicine already lowers pressure, please check with your pharmacist or doctor before adding nattokinase rather than starting it on your own.
