Why this applies to Antithrombin III

Antithrombin III falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Octacosanol interacts with.

Theoretically, octacosanol might inhibit platelet aggregation; however, clinical research suggests that this effect may not be clinically significant.
Octacosanol is the main component of policosanol. Some clinical research shows that taking policosanol 10-50 mg daily for 7-15 days can inhibit platelet aggregation in healthy patients. However, one clinical trial shows that taking policosanol 10 mg twice daily for 2 weeks prior to initiating warfarin does not affect warfarin pharmacokinetics or warfarin response. Furthermore, a study in patients undergoing percutaneous coronary intervention with a drug-eluting stent found that taking policosanol 40 mg plus clopidogrel and aspirin daily for 30 days modestly reduced the risk for minor bleeding events when compared with taking clopidogrel and aspirin alone.

Here's the simple version. Antithrombin III (Thrombate III) is an IV blood thinner given in a hospital to help prevent dangerous clots, often around surgery or for people with an inherited antithrombin deficiency. Octacosanol (the main piece of policosanol) has been looked at for possibly making platelets a little less sticky, which in theory could add to a blood-thinning effect and slightly raise bleeding risk.

The good news: the human evidence here is weak and mixed, and any platelet effect appears small and probably not clinically meaningful for most people. Still, because Thrombate III is a serious clot-prevention drug used in carefully monitored situations, don't add octacosanol on your own. Let the team managing your treatment know about any supplement so they can watch for easy bruising or unusual bleeding.
