Octacosanol is the main component of policosanol. In humans, policosanol can decrease blood glucose. It is not clear if these effects occur with the use of octacosanol.

Here's the simple version: repaglinide already works by pushing your pancreas to release more insulin, which lowers your blood sugar. Octacosanol, mostly because it's the main piece of policosanol, has been seen in some people to nudge blood sugar down a little too. Put the two together and the worry is an additive drop, meaning your sugar could fall lower than you expect.

If that happens, you might notice the usual signs of low blood sugar: shakiness, sweating, a fast heartbeat, dizziness, or feeling foggy and irritable. Honestly though, the evidence here is thin and somewhat theoretical, so this isn't a reason to panic. If you'd like to try octacosanol, keep an eye on your home glucose readings and check with your pharmacist or doctor first so nobody changes your repaglinide dose without a plan.
