Octacosanol and Streptokinase

Here is the interaction we found between these two, with the evidence behind it.

Moderate

ANTICOAGULANT/ANTIPLATELET DRUGS

Why this applies to Streptokinase

Streptokinase falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Octacosanol interacts with.

Theoretically, octacosanol might inhibit platelet aggregation; however, clinical research suggests that this effect may not be clinically significant.
Octacosanol is the main component of policosanol. Some clinical research shows that taking policosanol 10-50 mg daily for 7-15 days can inhibit platelet aggregation in healthy patients. However, one clinical trial shows that taking policosanol 10 mg twice daily for 2 weeks prior to initiating warfarin does not affect warfarin pharmacokinetics or warfarin response. Furthermore, a study in patients undergoing percutaneous coronary intervention with a drug-eluting stent found that taking policosanol 40 mg plus clopidogrel and aspirin daily for 30 days modestly reduced the risk for minor bleeding events when compared with taking clopidogrel and aspirin alone.

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Streptokinase is a powerful clot-busting (thrombolytic) drug, given in the hospital for emergencies like a heart attack, stroke, or a major clot in the lungs. Its biggest known risk is bleeding. The theory here is that octacosanol (the main part of policosanol) might make your platelets a little less sticky, which could in theory add to that bleeding risk.

The good news: the human evidence is weak and mixed, and studies suggest any blood-thinning effect is mild and may not be clinically meaningful. Since streptokinase is only used in urgent, closely monitored settings, this isn't something you'd manage on your own. Always tell the medical team about any supplements you take, and check with your pharmacist or doctor before starting octacosanol.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

From our pharmacist’s overview on the Streptokinase page

“Streptokinase (Kabikinase, Streptase) is the clot-buster given in emergencies like heart attacks, and because its whole job is dissolving clots, this list warrants real respect. Of the 219 supplement and herbal ingredients with interactions on file, 202 have blood-thinning or antiplatelet activity that can push bleeding risk higher.”

About Octacosanol

What is Octacosanol?

Octacosanol
  • Physical performance and endurance
  • Cholesterol support
  • Parkinson's and movement concerns
  • Antioxidant support
  • General energy and stamina

Octacosanol is a waxy long-chain alcohol from plant sources that is most often promoted for exercise endurance and cholesterol support, but solid human evidence is limited and results are mixed. It appears generally well tolerated in supplement amounts, but it is not a proven treatment for any condition, so talk with your pharmacist or doctor before using it.
