Why this applies to Aliskiren

Aliskiren falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

If you take ginseng along with aliskiren (Tekturna), the concern is fairly mild and mostly on paper at this point. Aliskiren is partly handled by a liver enzyme called CYP3A4, and Panax ginseng can nudge that enzyme in either direction. In studies it has sped some drugs up and slowed others down, so we can't say for sure which way aliskiren would go.

In practice, that means your blood pressure control could shift a little. If ginseng lowered aliskiren levels, your pressure might creep up; if it raised them, you could see more dizziness or lightheadedness. This is theoretical and not well studied for aliskiren specifically, so don't be alarmed. Just check with your pharmacist or doctor before adding ginseng, and keep an eye on your home blood pressure readings.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.
