Clarithromycin falls under QT INTERVAL-PROLONGING DRUGS — the group Panax Ginseng interacts with.
Clinical research shows that short-term use of Panax ginseng can increase the QT interval. However, no changes in QT interval have been identified with prolonged use.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: Clarithromycin can stretch out a part of your heartbeat called the QT interval, and so can Panax ginseng. When two things that do the same thing are taken together, the effect can add up. In rare cases, a longer QT interval can lead to an abnormal, fast heart rhythm. The good news is this concern is mostly theoretical, and the research is mixed. Short-term ginseng use nudged the QT interval a bit in studies, but longer use didn't.
This isn't about ginseng changing how clarithromycin is broken down. It's just two QT effects in the same place. The risk is higher if you already have heart rhythm issues, low potassium or magnesium, or take other QT drugs. Please run this combo by your pharmacist or doctor before starting ginseng.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Clarithromycin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the situation with clarithromycin and Panax ginseng. Your body uses an enzyme called CYP3A4 to process clarithromycin, and ginseng may nudge that enzyme in either direction depending on the person. If ginseng speeds the enzyme up, clarithromycin could clear faster and work a little less well against your infection. If it slows the enzyme down, you might see more of the usual side effects, like stomach upset, nausea, or that odd metallic taste clarithromycin can leave.
Honestly, this is mostly theoretical, and the human evidence is limited and mixed. For a short antibiotic course, I'd lean toward simply holding off on ginseng until you're done. If you take it regularly, give your pharmacist or doctor a quick call so they can weigh in on your specific case.

What Is Clarithromycin Used For?
Brand names: Biaxin, Biaxin XL
Clarithromycin tablets and oral suspension treat a range of mild to moderate bacterial infections in adults and children, including community-acquired pneumonia, acute sinus infections, acute bacterial flare-ups of chronic bronchitis, strep throat and tonsillitis, uncomplicated skin infections, and middle ear infections in children. Tablets are also used in adults to eliminate H. pylori bacteria (in combination with other medicines) to reduce the risk of duodenal ulcer recurrence.
Clarithromycin tablets — but not the extended-release tablet — are also used to treat and prevent disseminated Mycobacterium avium complex (MAC) infections in people with advanced HIV. Clarithromycin extended-release tablets are approved only for acute bacterial exacerbation of chronic bronchitis, acute maxillary sinusitis, and community-acquired pneumonia in adults — not for the other conditions listed above.
