Why this applies to Tadalafil

Tadalafil falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the deal with combining ginseng and tadalafil (Cialis). Your body clears tadalafil using a liver enzyme called CYP3A4, and Panax ginseng can nudge that enzyme in either direction. The science here is honestly pretty mixed and mostly theoretical, so I don't want you to lose sleep over it.

If ginseng slows that enzyme, tadalafil could build up a bit, meaning more headache, flushing, nasal stuffiness, back pain, or low blood pressure that leaves you lightheaded. If it speeds the enzyme up instead, tadalafil might work less well. Tadalafil is not a prodrug, so it works on its own. Occasional culinary amounts likely won't matter much, but concentrated ginseng supplements are worth running past your pharmacist or doctor first.

What Is Tadalafil Used For?

Brand names: Cialis, Tadliq

Tadalafil tablets (including Cialis and generic tadalafil) are used to treat erectile dysfunction (ED), the symptoms of benign prostatic hyperplasia (BPH — an enlarged prostate), or both conditions together. When used with the drug finasteride to start BPH treatment, this combination is recommended for up to 26 weeks.

Adcirca, Alyq (tablets), and Tadliq (oral suspension) are indicated specifically for pulmonary arterial hypertension (PAH — high blood pressure in the lungs), to improve the ability to exercise.
