Acarbose falls under ANTIDIABETES DRUGS — the group Pata De Vaca interacts with.

Animal research suggests that pata de vaca may have hypoglycemic effects. However, preliminary clinical research shows that pata de vaca does not reduce blood glucose levels. Until more is known, use with caution. Theoretically, concomitant use of pata de vaca with antidiabetes drugs might affect glucose control and increase the risk of hypoglycemia. Dose adjustments to diabetes medications might be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, metformin (Glucophage), pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the simple version. Acarbose works right in your gut, slowing how fast carbohydrates get broken down into sugar after a meal. Pata de vaca tea is traditionally used to help lower blood sugar too. So the concern is that stacking them could push your blood sugar lower than expected, which we call hypoglycemia. With acarbose specifically, signs to watch for include shakiness, sweating, dizziness, or feeling foggy.

I want to be honest about the evidence here: it's mostly theoretical and based on animal studies, and one small human study didn't even show a blood-sugar drop. So this isn't a red-alert situation. One quirk with acarbose: if a low does happen, treat it with glucose tablets or honey, not table sugar, since acarbose blocks regular sugar from absorbing quickly.
