Enasidenib falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Peppermint interacts with.
In vitro research shows that peppermint oil inhibits CYP2C19. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses an enzyme called CYP2C19 to help process enasidenib (Idhifa). In lab dish studies, peppermint oil appears to slow that enzyme down a bit. If that happened in real life, more enasidenib could linger in your system, which might make its effects and side effects (things like nausea, low appetite, or changes in your blood counts) a little stronger.
The important part: this is theoretical right now. It comes from lab testing only and hasn't actually been seen in people. A peppermint candy or a cup of mint tea is very unlikely to matter. Concentrated peppermint oil capsules taken regularly are a different story. Since enasidenib is a serious cancer medicine, please run any peppermint supplement past your oncologist or pharmacist first.

Here is the gist: your leukemia medicine Enasidenib (Idhifa) is partly handled by a liver enzyme called CYP2C9. In lab dish studies, peppermint oil slowed that same enzyme down. If that happened in the body, your liver might clear Enasidenib a little less quickly, letting levels nudge upward. In plain terms, that could mean a bit more of the drug's usual effects, things like nausea, diarrhea, fatigue, or changes in your bloodwork.
I want to be honest about the strength here: this is only a test-tube finding and has never actually been reported in a real person. A peppermint candy or cup of tea almost certainly won't matter. Concentrated peppermint oil capsules are the bigger question mark. Since Enasidenib is a serious cancer drug, just run any peppermint supplement past your oncology team first.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Clinical research in healthy volunteers shows that a single dose of peppermint oil 600 mg inhibits CYP3A4 enzymes and increases the AUC of felodipine, a CYP3A4 substrate. However, in vitro research suggests that peppermint oil only inhibits CYP3A4 at very high concentrations.

Here's the gist: peppermint oil (especially in concentrated supplement form) may gently block an enzyme in your body called CYP3A4. Since enasidenib (Idhifa) is partly cleared by that same enzyme, in theory the drug could build up a little higher than expected. With a cancer medicine like this, more drug in your system could mean a higher chance of side effects such as nausea, diarrhea, fatigue, or changes in your liver or blood counts.
I want to keep this in perspective though. This concern comes from limited research, and the effect on enzymes seems to show up mainly with concentrated oil doses, not a bit of peppermint tea or a mint. Still, because enasidenib is serious cancer therapy, please run any peppermint oil supplement by your oncology team first.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro and animal research shows that peppermint oil and peppermint leaf inhibit CYP1A2. However, in clinical research, peppermint tea did not significantly affect the metabolism of caffeine, a CYP1A2 substrate. It is possible that the 6-day duration of treatment may have been too short to identify a difference.

Here's the gist with peppermint and your leukemia medicine, enasidenib (Idhifa). Peppermint oil may gently slow down a liver enzyme called CYP1A2, which helps clear certain drugs. If that enzyme works more slowly, a drug it handles can build up a little, which could mean more side effects. For enasidenib, that might show up as nausea, decreased appetite, or changes in lab values your oncology team already watches closely.
The good news is that this is rated a minor, theoretical concern. It comes mostly from lab and animal studies, and in actual human testing peppermint tea didn't meaningfully change a CYP1A2 drug. Still, because enasidenib treats a serious illness, I'd mention any regular peppermint oil supplement use to your oncologist or pharmacist before starting it. An occasional cup of peppermint tea is very unlikely to matter.

Idhifa (enasidenib) is used to treat adult patients with acute myeloid leukemia (AML) — a fast-moving cancer of the blood and bone marrow — whose disease has come back after treatment (relapsed) or has stopped responding to treatment (refractory). It is specifically for patients whose leukemia has a mutation in the IDH2 gene, which must be confirmed by an FDA-approved test before starting therapy.
