Remdesivir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Phellodendron interacts with.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP3A4.
Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist: Phellodendron contains berberine, which lab and early human studies suggest can mildly slow down the CYP3A4 enzyme. Remdesivir is partly processed by that same enzyme, so in theory, blocking it could let a bit more of the drug build up in your system. That might mean a slightly higher chance of side effects like nausea or changes in liver enzymes.

That said, I want to keep this in perspective. This concern is theoretical, based on how these compounds behave in the lab rather than real-world reports. Remdesivir is also given in a hospital or clinic setting by IV, where you're monitored closely. Still, it's worth mentioning your Phellodendron use to the pharmacist or doctor managing your care so they have the full picture.

What Is Remdesivir Injection Used For?

Brand name: Veklury

Veklury (remdesivir) is given by IV infusion to treat COVID-19 caused by the SARS-CoV-2 virus. It is approved for adults and for children from birth through age 17 who weigh at least 1.5 kg (about 3.3 lbs).

Veklury can be used for patients who are hospitalized with COVID-19, or for patients who are not hospitalized but have mild-to-moderate COVID-19 and are at high risk of getting worse — including being hospitalized or dying from the disease.
