Alteplase, tPA falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Phellodendron interacts with.

Theoretically, phellodendron might increase the risk of bleeding when used with anticoagulant or antiplatelet drugs.
Phellodendron contains berberine. In vitro and in vivo research suggest that berberine can inhibit platelet aggregation. Theoretically, phellodendron might also inhibit platelet aggregation.

Here's the situation with alteplase (tPA). This is a powerful clot-busting medicine given in a hospital for things like a stroke, heart attack, or a serious lung clot. Because it dissolves clots, bleeding is already its biggest risk. The concern with Phellodendron is that the berberine it contains may make platelets a little less sticky, which could add to that bleeding risk on top of what alteplase already does.

I want to be honest about the evidence though: this is only theoretical, based on lab and animal studies, not real-world human reports. In practice, alteplase is given by a hospital team who carefully control everything around it. Still, it's smart to tell your doctors about any herbal supplement, including Phellodendron, so they have the full picture.
