Here's the thinking behind this pairing. Lenalidomide (Revlimid) works in part by calming or modulating certain immune signals as it fights conditions like multiple myeloma and myelodysplastic syndrome. Quillaia contains saponins, and in lab dish studies some of these compounds appear to stimulate the immune system. On paper, that nudge in the opposite direction could theoretically work against what lenalidomide is trying to do, which is the concern listed here.

I want to be honest though: this is a theoretical worry based only on test tube data, not real patient cases, so I wouldn't lose sleep over it. The amount of quillaia in foods and drinks (as a foaming agent) is unlikely to matter. Concentrated herbal supplements are a different story. Since lenalidomide is serious cancer therapy, please run any quillaia supplement past your oncology team or pharmacist first.

Quillaia (soap bark tree) is a plant whose bark contains foaming compounds called saponins, and it is widely used as an emulsifier in foods and drinks. As a supplement or herbal remedy its benefits are not well proven, and large amounts can be irritating or toxic, so it should be used cautiously and only as directed.

About Lenalidomide

What Is Lenalidomide Used For?

Brand name: Revlimid

Lenalidomide capsules (brand names Revlimid and Lenalidomide) are used to treat several blood cancers and related conditions in adults. These include multiple myeloma (MM) — both in combination with dexamethasone and as maintenance therapy after a stem cell transplant — and transfusion-dependent anemia caused by low- or intermediate-1-risk myelodysplastic syndromes (MDS) linked to a specific chromosome change called deletion 5q.

Lenalidomide capsules are also used for mantle cell lymphoma (MCL) that has come back or stopped responding after at least two prior treatments (one of which included bortezomib). In combination with a rituximab product, they treat previously treated follicular lymphoma (FL) and previously treated marginal zone lymphoma (MZL). Lenalidomide is not approved for chronic lymphocytic leukemia (CLL) outside of clinical trials.
