Here's the gist: Besremi (ropeginterferon) can be tough on the liver on its own, and interferons are known to sometimes raise liver enzymes. Rhubarb root, especially in concentrated supplement form, has been loosely tied to liver injury in a couple dozen reported cases, and animal studies hint its anthraquinones might stress the liver too. Putting two things that can each strain the liver together is the concern here.

I want to be honest, though: this is a theoretical worry based on pharmacology, not proven for this exact pairing. Watch for clues like yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness. Eating rhubarb stalks in a pie is very different from taking strong root supplements. Before adding any rhubarb product, check with your oncology team or pharmacist, since they're already monitoring your liver labs.

Brand name: Besremi

BESREMi (ropeginterferon alfa-2b) is used to treat two conditions in adults: polycythemia vera, a bone marrow disorder where the body makes too many red blood cells, and essential thrombocythemia, a condition where the bone marrow makes too many platelets.

Both conditions are given as subcutaneous (under-the-skin) injections of BESREMi, including the BESREMi Pen delivery option.
