Vadadustat falls under HEPATOTOXIC DRUGS — the group Rhubarb interacts with.

Theoretically, concomitant use of rhubarb with potentially hepatotoxic drugs might increase the risk of developing liver damage.
Some animal research suggests that anthraquinones in rhubarb might have hepatotoxic effects. Also, rhubarb use has been linked to at least 24 cases of liver injury, although details on the dose of rhubarb and duration of use in these cases is unclear.

Here's the concern with pairing rhubarb root supplements and Vadadustat (Vafseo), the medicine used to treat anemia from chronic kidney disease. Vadadustat is processed through the liver, and it already carries a known risk of raising liver enzymes and, rarely, causing liver injury. Rhubarb root contains compounds called anthraquinones that, in animal studies and a handful of human case reports, have been tied to liver stress of their own.

So the worry is simply additive: two things that can each be tough on the liver working at the same time. I want to be honest, though, this is theoretical and based on limited evidence, not a proven clash. Eating rhubarb stalks in a pie is very different from taking concentrated root capsules. Still, before adding any rhubarb supplement, please check with your pharmacist or kidney doctor.
