Saxagliptin, metformin XR falls under ANTIDIABETES DRUGS — the group Safflower interacts with.

Theoretically, safflower oil might alter the effects of antidiabetes drugs.
Some clinical research shows that taking safflower oil 10 grams daily for 3 weeks can increase fasting blood glucose in patients with type 2 diabetes. However, clinical research in patients with metabolic syndrome with or without impaired glucose tolerance shows that taking safflower oil 8 grams daily for 12 weeks reduces fasting glucose levels by around 8 mg/dL. Some clinical research also shows that taking safflower oil 8 grams daily for 16 weeks does not affect fasting glucose levels in patients with type 2 diabetes.

Here's the practical takeaway with Kombiglyze (saxagliptin plus extended-release metformin). The concern isn't about how your liver breaks this medicine down. Instead, concentrated safflower oil supplements may nudge your blood sugar one way or the other, which could add to or work against what your diabetes medicine is doing. The studies are honestly a bit of a mixed bag: one showed higher fasting glucose, another showed a small drop of about 8 mg/dL, and another showed no change at all.

Because the evidence pulls in different directions, I'd just be aware of it rather than alarmed. Cooking with safflower oil is very unlikely to cause trouble. If you start a daily supplement, it's smart to check your sugars a little more closely at first and run it by your pharmacist or doctor.
