Chlorpheniramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with sage and chlorpheniramine. Your body uses an enzyme called CYP3A4 to help break this antihistamine down. In a lab dish, water-based sage extracts seemed to slow that enzyme a bit. If that happened in real life, chlorpheniramine could linger a little longer and feel stronger, meaning more drowsiness, dry mouth, blurry vision, or grogginess.
The big caveat: this is only test-tube evidence, and nothing like it has actually been seen in people. Sprinkling sage on your dinner almost certainly won't matter. If you're taking strong sage capsules or the essential oil, just mention it to your pharmacist or doctor and watch for extra sleepiness rather than driving or doing anything risky until you know how you feel.

ANTICHOLINERGIC DRUGS

In vitro evidence suggests that common sage (Salvia officinalis) and Spanish sage (Salvia lavandulaefolia) can inhibit acetylcholinesterase and might increase acetylcholine levels.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this one. Chlorpheniramine works partly by blocking certain nerve signals (it has what we call anticholinergic activity). Sage seems to do the opposite in lab studies, it may raise levels of a brain chemical called acetylcholine. So in theory, sage could slightly push back against what chlorpheniramine is trying to do, meaning your allergy or cold symptoms (runny nose, sneezing, itchy eyes) might not be controlled quite as well.
I want to be honest about how shaky this is. It comes only from test-tube and lab research, not from people, so we really don't know if it matters in real life. A little sage in your cooking is almost certainly a non-issue. Concentrated sage extracts or capsules are where I'd be a bit more thoughtful. If you take these together regularly, just mention it to your pharmacist or doctor.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Chlorpheniramine is used to temporarily relieve common allergy symptoms caused by hay fever (allergic rhinitis) or other upper respiratory allergies. These include runny nose, sneezing, itchy or watery eyes, and itching of the nose or throat.
These uses are supported across multiple products — including tablets (such as Aller-chlor, Allergy, Allergy Relief, Allergy Time, Anti Allergy, and Chlorpheniramine Maleate), extended-release tablets (Allergy Relief and Chlorphen-12), the oral solution Ed Chlorped Jr, and the inhalers ClorRelief and ClorRelief KIDs.
