Dronabinol falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP2C9.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2C9. So far, this interaction has not been reported in humans.

Here's the gist: your body uses a liver enzyme called CYP2C9 to help clear dronabinol. Lab studies (in test tubes, not people) suggest that water-based sage extracts can slow that enzyme down. If sage blocks it, dronabinol could linger a bit longer and build up, which might mean stronger effects and more side effects like dizziness, drowsiness, a racing heartbeat, confusion, or feeling "high" or off-balance.

I want to be honest with you, though. This is only a theoretical concern based on lab work, and it has never been reported in real patients. Everyday cooking amounts of sage are very unlikely to matter. Concentrated sage capsules or oils are where I'd pause. If you take those regularly, just run it by your pharmacist or doctor first.

Here's the gist for Dronabinol: your body clears it largely through a liver enzyme called CYP3A4. In lab tests, water-based sage extracts seemed to slow that same enzyme down. If that happened in real life, dronabinol could linger a bit longer and hit harder, meaning more of its usual effects like drowsiness, dizziness, a racing heart, confusion, or feeling unusually "high." Dronabinol is not a prodrug, so blocking the enzyme tends to raise its effect, not weaken it.

The honest part: this is only a theory from test-tube work and hasn't been seen in actual patients. Sprinkling sage on dinner won't matter. Before using concentrated sage capsules or oil, just run it by your pharmacist or doctor.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
