Estradiol, Norgestimate falls under P-GLYCOPROTEIN SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase levels of drugs transported by P-glycoprotein.
In vitro research suggests that common sage (Salvia officinalis) can inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the gist for Ortho-Prefest, which pairs estradiol with the progestin norgestimate for menopause symptoms. Your body uses a tiny pump called P-glycoprotein to move certain drugs around. In test-tube studies, sage seems to slow that pump down. If the same thing happened in people, slightly more of your hormones could stick around, which might mean a bit more of the usual effects, like breast tenderness, nausea, bloating, headaches, or spotting.

Now the reassuring part: this is only laboratory evidence and has never been shown in real patients, so I wouldn't lose sleep over it. Cooking with sage is a non-issue. Concentrated sage extracts or the essential oil are where I'd be more cautious. If you take those regularly, just give your pharmacist or doctor a heads-up.

Here's the gist with Ortho-Prefest, which is a hormone product that pairs an estrogen (estradiol) with a progestin (norgestimate). The concern with sage is pretty theoretical. In lab dishes, a compound called geraniol found in Spanish sage seemed to act a little like estrogen itself. The worry is that adding estrogen-like activity on top of your hormone therapy could nudge your hormone effects slightly.

But honestly, the evidence here is weak and only from test-tube studies, not people. Cooking with sage in normal food amounts is very unlikely to do anything. Concentrated sage extracts or essential oil are where I'd be more cautious. If you want to take a sage supplement regularly, just run it by your pharmacist or doctor first.
