Why this applies to Pazopanib

Pazopanib falls under P-GLYCOPROTEIN SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase levels of drugs transported by P-glycoprotein.
In vitro research suggests that common sage (Salvia officinalis) can inhibit the multi-drug transporter protein, P-glycoprotein. This effect has not been reported in humans.

Here's the idea behind this one. Your body uses a kind of cellular pump called P-glycoprotein to move certain drugs out of cells. In test-tube studies, sage seems to slow that pump down. Since pazopanib (Votrient) is carried by P-glycoprotein, blocking the pump could, in theory, let a little more of the drug build up in your system.

If that happened, you might notice stronger side effects like high blood pressure, fatigue, diarrhea, nausea, or changes in liver tests. But I want to be honest about the evidence: this has only been seen in the lab, never confirmed in real people, and a sprinkle of sage on your dinner is very unlikely to matter. Concentrated sage extracts or sage oil are the bigger question. Before adding any sage supplement, please check with your oncology team or pharmacist.

Pazopanib also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the gist for Votrient (pazopanib): your liver uses an enzyme called CYP3A4 to clear it out. In lab tests, water-based sage extracts seemed to slow that same enzyme down. If that happened in real life, less pazopanib would be broken down, so more could build up in your body. With a cancer drug like this, higher levels can mean more side effects, things like high blood pressure, diarrhea, nausea, fatigue, or, less commonly, liver or heart-rhythm problems.

The honest part: this is only a theoretical concern from test-tube studies, and it hasn't been seen in people. A little sage in your cooking is very unlikely to matter. Concentrated sage capsules or oil are a different story, so please run those past your oncologist or pharmacist before adding them.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Pazopanib Used For?

Brand name: Votrient

Pazopanib tablets — sold as the brand Votrient and as generic pazopanib — are used to treat two types of advanced cancer in adults: advanced renal cell carcinoma (RCC), the most common form of kidney cancer, and advanced soft tissue sarcoma (STS) in patients who have already received prior chemotherapy.

Important limitation: Pazopanib has not been shown to work in adipocytic soft tissue sarcoma (a subtype involving fat cells) or in gastrointestinal stromal tumors (GISTs), so it is not used for those cancers.
