Temsirolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP3A4.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP3A4. So far, this interaction has not been reported in humans.

Here's the gist for Temsirolimus (Torisel). Your body uses a liver enzyme called CYP3A4 to help clear this cancer drug. Lab studies hint that sage, especially concentrated extracts, may slow that enzyme down a bit. If that happened in a real person, temsirolimus could build up and its side effects could get stronger, things like mouth sores, rash, fatigue, swelling, high blood sugar, or low blood counts that raise infection risk.

The important part: this is theoretical and only seen in test tubes, never reported in actual patients. A little sage on your dinner is very unlikely to matter. Concentrated capsules or sage oil are the bigger question. Since this is a serious cancer medicine, please run any sage supplement past your oncology team or pharmacist first.
