Here's the simple version: sage contains some natural compounds that can have a mild calming, sleepy effect on the brain. Topiramate already tends to slow brain activity, which is how it helps with seizures and migraines, but that's also why it can leave you feeling drowsy, foggy, dizzy, or slowed down in your thinking. So in theory, pairing concentrated sage with topiramate could stack those effects and make you feel more tired or out of it.

One important point: this concern comes only from lab and animal research, not from real people, so it's far from certain. A sprinkle of sage in cooking is very unlikely to matter. Stronger sage extracts or essential oil are where I'd be more cautious. If you use sage supplements, just mention it to your pharmacist or doctor first.

Here's the simple version: if you take topiramate (Topamax) to control seizures or prevent migraines, the worry with sage is that certain sage species, especially concentrated extracts and essential oils, contain a compound called thujone that can actually lower the seizure threshold in large amounts. In theory, that could work against what topiramate is trying to do and make a seizure more likely to break through.

I want to be honest about the evidence, though. This concern comes from lab and animal studies, not from people, so it's mostly theoretical. A pinch of sage in your cooking is very unlikely to cause any trouble. The bigger caution is with strong sage supplements, extracts, or the essential oil. Before adding any of those, check with your pharmacist or doctor so we keep your seizure control steady.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.

What Is Topiramate Used For?

  • Topiramate is used alone or with other medications to treat certain types of seizures including primary generalized tonic-clonic seizures (formerly known as a grand mal seizure; seizure that involves the entire body) and partial onset seizures (seizures that involve only one part of the brain). Topiramate is also used with other medications to control seizures in people who have Lennox-Gastaut syndrome (a disorder that causes seizures and developmental delays). Topiramate is also used to prevent migraine headaches but not to relieve the pain of migraine headaches when they occur. Topiramate is in a class of medications called anticonvulsants. It works by decreasing abnormal excitement in the brain.
