Strawberry and Ivermectin

Here is the interaction we found between these two, with the evidence behind it.

Moderate

P-GLYCOPROTEIN SUBSTRATES

Why this applies to Ivermectin

Ivermectin falls under P-GLYCOPROTEIN SUBSTRATES — the group Strawberry interacts with.

In vitro research suggests that strawberry extract can inhibit p-glycoprotein efflux. Theoretically, strawberry might inhibit p-glycoprotein mediated drug efflux and potentially increase levels of drugs that are substrates of p-glycoprotein. Until more is known, strawberry should be used cautiously in people taking p-glycoprotein substrates.
Drugs that might be affected include some chemotherapeutic agents (etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the short version: your body uses a tiny pump called p-glycoprotein to push ivermectin back out and keep it from building up too much, including limiting how much reaches your brain. In lab dish studies, strawberry extract slowed that pump down. If that happened in a person, a bit more ivermectin could hang around than expected, which in theory could mean more side effects like dizziness, drowsiness, nausea, or headache.

The honest part: this is only test-tube evidence, not something proven in people. Eating strawberries as a normal food is very unlikely to cause any trouble. Concentrated strawberry supplements are the bigger question mark. Since ivermectin is usually a short course, I'd just mention any strawberry supplements to your pharmacist or doctor before starting.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

Strawberry is a popular, nutrient-rich fruit that supplies vitamin C, fiber, and antioxidant plant compounds. About Ivermectin

What Is Ivermectin Used For?

Brand names: Stromectol, Mectizan, Sklice, Soolantra

Ivermectin tablets (including Stromectol) are used to treat two parasitic infections: strongyloidiasis (a roundworm infection of the intestinal tract) and onchocerciasis (river blindness, caused by a parasitic worm spread by blackfly bites). These are prescription uses for serious infections.

Topical ivermectin lotion — sold under several names including RID One and Done, Head Lice Treatment Ivermectin Lotion, Scalp Care Lotions, and others — is applied to the scalp to treat head lice. A separate topical ivermectin cream (1%), available as Soolantra and ivermectin cream 1%, is used to treat the inflammatory pimple-like bumps of rosacea.
