Why this applies to Vincristine

Vincristine falls under P-GLYCOPROTEIN SUBSTRATES — the group Sweet Orange interacts with.

Sweet orange juice seems to modulate P-glycoprotein (P-gp), which might affect the blood levels of P-gp substrates.
Animal and in vitro research suggest that orange juice extract inhibits drug efflux by P-gp, increasing absorption and levels of P-gp substrates. In contrast, pharmacokinetic research in humans shows that drinking large amounts of sweet orange juice decreases absorption and levels of the P-gp substrate celiprolol. This suggests that orange juice actually induces drug efflux by P-gp or affects drug levels by another mechanism such as inhibiting the gut drug transporter called organic anion transporting polypeptide (OATP). Until more is known, sweet orange juice should be used cautiously in people taking P-gp substrates.

Here's the situation with sweet orange and your vincristine. Vincristine is pumped around your body partly by a transporter called P-glycoprotein, and sweet orange juice appears to nudge that pump. The tricky part is that the research points in two directions: lab and animal studies suggest orange juice could raise drug levels, while human studies of a similar pump-handled drug showed it actually lowered levels. So we honestly can't say for sure which way it would push vincristine.

Because vincristine is potent chemo (with real risks like nerve tingling, constipation, and low blood counts), even small shifts matter. This isn't strong evidence, and an orange now and then is unlikely to be the problem. Still, please run any regular orange juice or concentrated orange supplements past your oncology team first.

What Is Vincristine Injection Used For?

Vincristine Sulfate Injection is approved to treat acute leukemia. It is also used as part of combination chemotherapy regimens for Hodgkin's disease, non-Hodgkin's malignant lymphomas, rhabdomyosarcoma (a soft-tissue cancer), neuroblastoma (a cancer of nerve tissue), and Wilms' tumor (a kidney cancer in children).

Because it works best alongside other cancer medicines, vincristine is almost always given as part of a multi-drug treatment plan rather than on its own.
