Here's the concern with this pairing. Your body clears vincristine using a liver enzyme called CYP3A4. St. John's wort revs that enzyme up, so it can flush vincristine out of your system faster than intended. The worry isn't more side effects, it's the opposite: lower drug levels could mean your chemotherapy doesn't work as well as it should against the cancer it's meant to fight.

Vincristine isn't a prodrug, so this is a real loss-of-effect issue, not an activation one. The evidence here is moderate, but with cancer treatment we don't take chances. Please don't start or stop anything on your own. Loop in your oncologist or pharmacist before taking St. John's wort so your treatment stays on track.

St. John's wort decreases the levels and clinical effects of P-glycoprotein substrates.
St. John's wort induces P-glycoprotein. P-glycoprotein is a carrier mechanism responsible for transporting drugs and other substances across cell membranes. When P-glycoprotein is induced in the gastrointestinal (GI) tract, it can prevent the absorption of some medications. In addition, induction of p-glycoprotein can decrease entry of drugs into the central nervous system (CNS) and decrease access to other sites of action.

Here's the concern with combining St. John's wort and vincristine. This herb speeds up a transporter called P-glycoprotein, which acts like a pump that moves drugs out of cells. Vincristine relies on staying inside cancer cells to do its job, and it's also cleared by liver pathways that St. John's wort can ramp up. So the worry is that the herb may lower vincristine levels, meaning your chemotherapy could work less effectively against the cancer it's targeting.

With a powerful cancer drug like this, you really don't want to risk weakening it. Please don't start or stop anything on your own. Talk with your oncology team or pharmacist first, since they coordinate every part of your chemo plan.

What Is Vincristine Injection Used For?

Brand name: Vincasar

Vincristine Sulfate Injection is approved to treat acute leukemia. It is also used as part of combination chemotherapy regimens for Hodgkin's disease, non-Hodgkin's malignant lymphomas, rhabdomyosarcoma (a soft-tissue cancer), neuroblastoma (a cancer of nerve tissue), and Wilms' tumor (a kidney cancer in children).

Because it works best alongside other cancer medicines, vincristine is almost always given as part of a multi-drug treatment plan rather than on its own.
