Here's the concern with combining Syrian Rue and your blood pressure combo pill (hydrochlorothiazide with methyldopa). Both of these are mainly about the liver, but in a different way than enzyme effects. Methyldopa is a well-known cause of liver injury in a small number of people, and Syrian Rue seeds, especially in large amounts, have been linked to liver damage too. Stacking two things that can stress the liver may raise the odds of trouble.

To be fair, this is a theoretical concern based on how these act, and the worrying liver cases involved huge amounts of seeds (up to 100 grams). Still, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and check with your pharmacist or doctor before mixing them.
