Hydrocodone Bitartrate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Syrian Rue interacts with.
Syrian rue contains beta-carboline alkaloids, including harmine, harmaline, harmalol, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids can inhibit CYP2D6 enzymes.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the wrinkle with hydrocodone: your body uses an enzyme called CYP2D6 to convert it into hydromorphone, a more potent painkiller. Syrian Rue's harmala compounds appear to block CYP2D6 (at least in lab studies), so in theory less of your hydrocodone would get turned into that stronger form. That could mean you feel less pain relief than expected.
The flip side is that more of the parent hydrocodone may hang around, and these things are hard to predict in a real person. Right now this is only a theoretical, lab-based concern, not something proven in patients. Still, Syrian Rue carries its own real toxicity risks, so please don't combine them without checking with your pharmacist or doctor first.

Syrian rue contains beta-carboline alkaloids, including harmine, harmol, and harmane. In vitro research suggests that these beta-carboline alkaloids competitively inhibit CYP3A4 enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with these two together. Your body uses an enzyme called CYP3A4 to help clear hydrocodone. Lab studies suggest the harmala alkaloids in Syrian Rue can slow that enzyme down. If that happens in real life, more hydrocodone could linger in your system, which may push its effects and side effects higher (think extra drowsiness, dizziness, constipation, and in a serious case, slowed or shallow breathing).
I want to be honest about the strength of this: it's theoretical, based on test-tube data, not solid human studies. Still, hydrocodone is a strong opioid where breathing slowing down is a real concern, so I'd play it safe. Syrian Rue also acts as an MAO inhibitor, which adds its own risks. Please loop in your pharmacist or doctor before combining them.
