Tangerine and Hydrocodone, Pseudoephedrine, Chlorpheniramine

Here is the interaction we found between these two, with the evidence behind it.

Minor

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Hydrocodone, Pseudoephedrine, Chlorpheniramine

Hydrocodone, Pseudoephedrine, Chlorpheniramine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Good news first: this is a very minor concern, and for most people eating or drinking tangerine while taking Zutripro (hydrocodone, pseudoephedrine, chlorpheniramine) is unlikely to cause a problem. The theory comes from lab studies suggesting a tangerine compound called tangeretin might slightly speed up the CYP3A4 enzyme. Hydrocodone partly relies on that pathway, and it's actually a prodrug, meaning the body has to convert part of it into its active form.

So in plain terms, even if tangerine nudged that enzyme along, the practical effect on your pain relief or side effects (drowsiness, dizziness, dry mouth) would likely be tiny. Importantly, the one human study found tangerine juice did not meaningfully change drug metabolism. Enjoy your tangerine, but if you have questions about your dose, check with your pharmacist or doctor.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.

About Tangerine

What is Tangerine?

  • Source of vitamin C
  • Digestive support
  • Aromatherapy and relaxation
  • Cough and phlegm (traditional)
  • Skin and flavoring uses

Tangerine is a sweet citrus fruit that is a good source of vitamin C and other nutrients, and is widely enjoyed as food. While the peel and essential oil are used in traditional medicine and aromatherapy, strong human evidence for specific health benefits beyond basic nutrition is limited. Enjoying tangerines as part of a balanced diet is generally safe for most people.
