Why this applies to Ruxolitinib

Ruxolitinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the short version: your body uses an enzyme called CYP3A4 to break down ruxolitinib (Jakafi, Opzelura). A compound in tangerine called tangeretin sped that enzyme up a little in lab tests, which in theory could clear ruxolitinib faster and make it slightly less effective. With a blood-cancer medicine, weaker effect isn't something to shrug off.

That said, this is mostly test-tube data. When people actually drank tangerine juice, it did not meaningfully change how a similar drug was processed. So enjoying tangerines as a normal part of your diet is very unlikely to cause a problem. I'd just mention any large or concentrated citrus supplements to your pharmacist or oncology team so they can keep an eye on things.

What Is Ruxolitinib Used For?

Brand names: Jakafi, Opzelura

Ruxolitinib is approved to treat several serious blood and immune conditions in adults, and some in patients as young as 12. It is used for myelofibrosis (a bone marrow disorder that disrupts normal blood cell production), polycythemia vera (a condition where the bone marrow makes too many red blood cells), and two forms of graft-versus-host disease (GVHD) — a complication that can occur after a stem cell transplant.

For polycythemia vera, it is specifically for patients who have not responded well to, or cannot tolerate, a medication called hydroxyurea. For chronic GVHD, it is used after at least one or two other treatments have failed.
