Why this applies to Sotorasib

Sotorasib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Tangerine interacts with.

In vitro, tangeretin, a constituent of tangerine, induces a 52% increase in the metabolism of midazolam by cytochrome P450 3A4 (CYP3A4). This suggests that tangeretin may stimulate CYP3A4 activity. However, in humans, drinking tangerine juice 200 mL slightly delayed the absorption, but did not affect the metabolism, of midazolam, a CYP3A4 substrate. Theoretically, tangerine juice might increase CYP3A4 activity and decrease levels of drugs metabolized by this enzyme. However, this effect is unlikely.
Some drugs metabolized by CYP3A4 include amitriptyline (Elavil), amiodarone (Cordarone), citalopram (Celexa), felodipine (Plendil), lansoprazole (Prevacid), ondansetron (Zofran), prednisone (Deltasone, Orasone), sertraline (Zoloft), sibutramine (Meridia), and many others.

Here's the quick version: your lung cancer pill, sotorasib (Lumakras), is cleared from your body by a liver enzyme called CYP3A4. There's a lab finding that a compound in tangerine, tangeretin, can slightly speed up that enzyme. In theory, a faster enzyme could clear sotorasib a little quicker and make it less effective, which matters because you want full strength against the cancer.

The reassuring part: when this was actually tested in people using tangerine juice, it did not meaningfully change drug metabolism. So this is a minor, unlikely concern, and a tangerine or two with your meals shouldn't cause trouble. Still, since sotorasib is a serious cancer therapy, run any concentrated tangerine supplements or peel extracts past your oncology pharmacist first.

What Is Sotorasib Used For?

Brand name: Lumakras

Lumakras is used to treat adults with KRAS G12C-mutated locally advanced or metastatic non-small cell lung cancer (NSCLC) who have already received at least one prior cancer treatment. This use was approved under the FDA's accelerated approval pathway based on tumor response rates, and continued approval may depend on results from confirmatory trials.

Lumakras is also used in combination with panitumumab to treat adults with KRAS G12C-mutated metastatic colorectal cancer (mCRC) who have previously received certain chemotherapy regimens. In both cases, patients must first be confirmed to carry the KRAS G12C mutation using an FDA-approved test.
