Here's the simple version. When you take a thyroid extract supplement (which acts like thyroid hormone), it absorbs best on an empty stomach. Your digestive enzymes (amylase, lipase, protease, sold as products like Pangestyme) are meant to be taken right when you eat. So the real issue is timing and food, not a direct chemical battle between the two.

If you swallow both at the same meal, the food and stomach contents could blunt how much thyroid hormone gets absorbed, which over time might leave you feeling sluggish, cold, or tired. The good news is this is easy to manage: take the thyroid product on an empty stomach, then take your pancreatic enzymes with your actual meal later. Pancrelipase is used to treat exocrine pancreatic insufficiency (EPI) — a condition where the pancreas doesn't produce enough digestive enzymes. Without these enzymes, the body struggles to break down and absorb nutrients from food.

It is indicated for adults with EPI caused by chronic pancreatitis (long-term pancreas inflammation) or pancreatectomy (surgical removal of the pancreas, in whole or in part). It is also used in people with cystic fibrosis who develop EPI.
