Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here's the simple version for Lomotil (the diphenoxylate plus atropine combo you use for diarrhea): the concern on file is really about how well your body absorbs the thyroid hormone, not the other way around. When something is sitting in your stomach and gut, like another medicine or food, it can bind to thyroid hormone and keep some of it from soaking in. That can make your thyroid product a little less effective.

This is rated minor, so it isn't dangerous, just a timing issue. A practical fix is to take your thyroid product on an empty stomach and space it a few hours apart from your Lomotil. Since diarrhea itself speeds things through the gut and can cut absorption too, it's worth running your routine past your pharmacist or doctor.
