Nirmatrelvir, Ritonavir falls under Oral Drugs — the group Thyroid Extract interacts with.

Food and medications can interfere with thyroid hormone absorption. Some foods, particularly those high in fiber or containing certain minerals like calcium or iron, can bind to thyroid hormones and reduce their absorption in the digestive tract. Additionally, taking thyroid medications with certain drugs, like antacids or calcium supplements, can hinder the effectiveness of the thyroid medication, necessitating adjustments in dosage or timing. Prescription thyroid medications, such as Synthroid, are recommended to be taken on an empty stomach, at least 30 minutes before food and other medications, for optimal absorption.

Here the concern runs the other way from what you might expect. It is the thyroid extract that can get poorly absorbed, not your Paxlovid. Taking the two close together in your stomach may let your Paxlovid pill (or food taken with it) bind up some of the thyroid hormone, so you absorb less of it. Over time that could mean your thyroid symptoms creep back, things like tiredness, feeling cold, or weight changes.

The good news is this is a minor, timing-based issue, and Paxlovid is only a 5-day course. A simple fix is spacing your thyroid product away from other pills and food (usually on an empty stomach, well before). Run the exact timing by your pharmacist, and please use prescription thyroid medicine rather than an unstandardized extract.
